Black Widow Envenomation
Black Widow Envenomation
Patient with highly suspected or confirmed Black Widow Spider Bite
Symptoms requiring parenteral medications (usually pain, spasm, etc)
Stable Vital Signs (mild hypertension and tachycardia are fine, and common but must be stable)
Stable mental status, some mild sedation ok (RAAS -2 or greater)
Unstable vital signs
Decreased mental status
Respiratory Compromise
Cardiac and pulmonary monitoring
Update TDAP
IV opiates and Benzodiazepines for pain and spasm
Consult poison control if symptoms worsening, uncontrolled or prolonged beyond 24 hours.
EtCO2 if warranted
Regular Diet
Disposition
Symptoms improved / controlled on oral medications after 24 hours
No respiratory issues
Worsening Symptoms after 24 hours
Anticipated need for Antivenem
Patient unable to take PO medications or able to safely care for self in home environment.
Patient unable to ambulate
