Wound Care Evaluation
Patient with a chronic wound, or acute/chronic pressure ulcer that needs wound care recommendations
Does not have sepsis physiology
Does not need sharp debridement from Surgery
Concern for deep space infection
Large area of eschar (likely to need sharp debridement)
Sepsis physiology
Not able to return to current living situation after wound care consultation. (May need transfer to center with comprehensive wound center, University, PSL)
Wound Care Evaluation and Treat
Care Management Orders
Local wound care by nursing as suggested by Wound Care RN Eval
Surgery Consultation as needed
Disposition
Wound Care Eval Completed
Outpatient wound care services and supplies arranged
Stable Vital signs
Unstable vital signs or worsening clinical condition
Significant lab or imaging abnormalities
Patient unable to take PO medications
Wound care and CM determine patient needing placement for treatment of wounds
